Summary: Facilitates concurrent documentation of the medical record to achieve accurate inpatient coding and DRG assignments to ensure the principal diagnosis, comorbidities and principal procedure are appropriate and supported for reimbursement. Reviews facility case mix financial data to identify trends and problem areas. Directly works with Physician Advisors as well as the patient’s attending physician to establish concise physician documentation; ensures physician awareness of documentation guidelines in relation to coding guidelines that supports actual care provided during the hospitalization. Assists Director with denial appeal process, external reviews, Documentation Improvement Program monitoring, auditing, training, interviewing, test updates, policy/procedure creation and updating, etc. Audits documentation and DRG assignment for accuracy and opportunities for improvement and assists with determining and implementing education of findings (on routine audits of DRG Clinical Documentation Specialists as well as on an as needed basis for specific audits). Attends meetings regarding DRG, Coding and documentation issues/concerns with ancillary departments, service line directors, medical staff chairs and Vice Presidents. Formal Education Required: • Completion of coding certification program and RHIA, RHIT, CCDS, CDIP, CCS or CPCH certification OR Graduate of an accredited school or professional nursing or a school accepted for candidacy status from an accrediting body with current Nursing license in the State of Ohio. • RHIA, RHIT, CCS, CDIP, CCS or CPCH certification preferred OR Associate in Health Information Technology Experience and Training Required: - Three years of inpatient clinical documentation specialist experience in an acute care facility and three (3) years of inpatient coding experience in an acute care facility with a passing grade of 85% or better on a DRG Clinical Documentation Specialist Test if minimum education level is coding certificate or Associate in Health Information Technology. • Three years of inpatient clinical documentation specialist experience in an acute care facility and three (3) years professional nursing experience which should include a combination of medical, specialty and/or surgical care in an acute care setting and a passing grade of 85% or better on a DRG Clinical Documentation Specialist Test required if minimum education level is graduate of nursing school.
