RN Transitional Care Coordinator/Case Manager Full time 36 hrs/week 12 hr shifts

Facilitates development, implementation, coordination, and evaluation of patient’s individualized plan which starts on admission and involves and prepares the patient/family for self-management post stay. Collaborates with patient, family, physician and interdisciplinary team to assure care is provided from acute care through the continuum. Includes taking a leadership role with the interdisciplinary team to achieve actions and processes designed to ensure optimal clinical outcomes, coordination and continuity of health care as patients transfer between different venues of care and promotes effective utilization of healthcare resources through monitoring the patient's stay.
1.Formal Education Required:
-Graduate of an accredit BSN school of professional nursing or a school accepted for candidacy status from an accrediting body
-Registered Nurse, Licensed in the State of Ohio
2.Experience & Training Required:
-Two (2) years recent experience in clinical, utilization management, home care, discharge planning, and/or case management

-Demonstrates a high level of clinical competence that is recognized by peers
-Is knowledgeable of disease processes, treatment protocols and pharmacological management
-Demonstrated ability to communicate clearly and sensitively in writing and orally with physicians, patients and families, health care professionals, and others
-Ability to interact and collaborate with physicians and other health care professionals in a professional manner
-Ability to organize and coordinate the work efforts of others
-Ability to prioritize work and solve problems
-Assumes accountability for demonstrating behaviors consistent with the customer service policy
-Population Specific Competency: Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity
-Maintains current Basic Life Support certification
4.Level of Physical Demands:
-Heavy: Exerts 50-100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or ten to 20 pounds of force continuously to move objects.
